Syrene was excited to be participating in a program that people the basics of properly tending the sick and injured. She had recently become interested in medicine, and this was the first step towards becoming a full-fledged medic. Syrene arrived at the hospital and joined five other people who were taking the course with her. Their instructor was a middle-aged nurse that led them to a room with a lot of medical equipment. The woman stood at the front of the room and said, “My name is Nurse Clara and today I’m going to be teaching you how to check pulses.”
Knowing how to check pulses is important in determining whether someone is dead or merely unconscious, among other things. We’re going to start with the carotid pulse which is in the neck. Pair up and practice with each other. There are several charts scattered across the room if you need more information. I’ll also be here to answer any questions. There’s one more thing you need to tell you before you begin. There is a carotid artery on each side of the neck, but don’t press them at the same time because you will block the blood flow to the brain.”
Syrene carefully listened to Nurse Clara’s instructions before scanning the room for a chart that showed the arteries in the head and neck. Another girl in the course, Vivian, had the same idea as Syrene, so the two of them decided to pair up. After studying the chart to get a feel for the general area she was supposed to be looking in, Syrene tried to find Vivian’s carotid pulse. She used her first two fingers to feel along Vivian’s neck. It took her a couple of tries before she was finally able to find it. Syrene continued feeling along the area where the chart said the carotid artery was located to see if she could follow it down to Vivian’s shoulder.
Syrene was able to follow the artery, but she noticed that the pulse was strongest right where the neck and head joined. Syrene was easily able to find the carotid pulse on the other side of Vivian’s neck before Vivian practiced finding Syrene’s pulse. When they were finished, Nurse Clara came over and asked for a demonstration. Then she taught them how to find the radial pulse. “The radial pulse is located in the arm right under the thumb. It’s a little harder to find than the carotid pulse, but the method is the same.”
Syrene and Vivian located another chart that showed them the artery they were looking for. Once again, Syrene used her first two fingers and gently pressed into Vivian’s skin. It took Syrene several tries to find the radial pulse sense it was much weaker than the carotid pulse. Syrene discovered that the pulse was much easier to detect when Vivian’s arm was resting on something. Syrene found the radial pulse on Vivian’s other arm before letting Vivian practice on her.

• Novice: Diagnose; Tend to injured characters such that their wounds heal at a faster rate; critical care requires 10 minutes/wound; can distinguish whether an individual is dead or merely unconscious.
• Average: Stabilize a character knocked unconscious from trauma; critical care requires 6 minutes/wound; with a successful Average skill check to resuscitate a dying character.
• Advanced: Function as a midwife to deliver children; diagnose injuries; critical care requires 4 minutes/wound
• Expert: Properly set broken bones; amputate limbs; critical care requires 3 minutes/wound
• Master: Critical care needs 2 min. per wound
